Nous recrutons pour un établissement de santé partenaire situé dans le nord des Deux-Sèvres, spécialisé dans la gériatrie. Nous proposons un CDD de 6 à 9 mois pour remplacer un congé maternité. Le poste est à pourvoir au sein du service SSR où vous serez en charge d'élaborer et de réaliser des programmes d'interventions, tant individuels que collectifs, axés sur la prévention et la rééducation active. Vous aurez également pour mission de préparer les retours à domicile des patients et de participer aux réunions pluridisciplinaires. Le candidat idéal doit être titulaire d'un master APAS-ERI. Ce poste est à temps plein, avec une charge de travail de 35 heures par semaine. Les candidatures doivent être adressées directement à Mediirect.
