Les missions incluent : - Temps thérapeutique : accompagnement des résidents, des familles et des aidants dans différents services d'hébergement, PASA, dispositif "Noctambule", accueil de jour et relayage à domicile. - Temps d'évaluation : réalisation d'échelles gériatriques. - Temps institutionnel : collaboration et échanges avec les équipes pluridisciplinaires (transmissions, réunions). - Temps organisationnel : gestion des orientations sur l'Accueil de Jour, coordination de l'équipe de l'Accueil de Jour et relayage.
