**Résumé de l'Offre d'Emploi : Pharmacien Référent des Services Cliniques** Recrutement pour un établissement partenaire de renom en région parisienne. Nous recherchons un pharmacien hospitalier pour un poste à temps plein (9h-18h) assurant des missions variées en gériatrie (onco-hématologie, cardiologie, neurologie, psychiatrie, soins palliatifs). **Principales Missions :** 1. Analyse pharmaceutique des prescriptions avec ORBIS®. 2. Gestion de l'approvisionnement et dispensation des dispositifs médicaux (DMS, DMI). 3. Qualité et gestion des risques, évaluation des pratiques professionnelles. 4. Participation aux comités et réunions hospitalières. 5. Remplacement du responsable d’UF en son absence. **Compétences et Pré requis :** * Autonomie, polyvalence, excellent relationnel, réactivité, organisation. * DES de pharmacie hospitalière, inscription à l'ordre des pharmaciens ou affectation CNG/ARS pour praticiens non-inscrits à l'ordre. **Projet Professionnels :** Participation aux projets de conciliation médicamenteuse, automatisation de la PUI, et validation des prescriptions d’anticancéreux.
