**Résumé de l'offre :** Un établissement partenaire recrute un gériatre/médecin généraliste inscrit à l'ordre pour un poste de médecin coordonnateur au sein d'un EHPAD de 84 places, dont 20 en unité Alzheimer. Ce poste à temps partiel (50-80%) implique l'évaluation, l'orientation et la coordination des soins pour les patients âgés. Le candidat travaillera en collaboration avec des professionnels libéraux et des réseaux médicaux (soins palliatifs, gériatrie, HAD). Il participera aux staffs pluridisciplinaires, aux travaux des filières gériatrique et AVC, ainsi qu'à la permanence des soins du pôle personnes âgées. Des compétences complémentaires en gériatrie et la capacité à sensibiliser les équipes aux spécificités du sujet âgé sont souhaitées.
