Nous recrutons pour un établissement médico-social public accueillant des enfants et adolescents handicapés de 6 à 20 ans. Ce dispositif, en devenir DIME/DAME, propose 48 places en accueil de jour et 24 places en milieu ordinaire. L'EEAP inclus 15 places pour adolescents polyhandicapés avec troubles associés, et 5 lits d'internat hebdomadaire. Missions : - Suivi médical et coordination avec les médecins psychiatres, MPR et médecins traitants. - Surveillance médicale générale des enfants et adolescents TND et polyhandicapés. - Examens médicaux annuels, suivi de santé, coordination des suivis médicaux. - Avis médicaux pour activités socio-éducatives et sportives. - Mise en place et suivi des prescriptions. - Relais avec équipes sanitaires. - Rencontre et information des familles. - Soutien des parcours de soins. - Évaluation des candidatures, rédaction de rapports médicaux. - Participation au rapport d'activité annuel.
