Vos missions incluent l'élaboration de projets individuels, la médiation avec les familles, la préparation des sorties et des admissions en structures, et l'instruction de dossiers divers. Vous interviendrez également dans la prévention et la protection de l'enfance, assurerez la coordination avec diverses institutions et faciliterez l'accès aux soins pour les patients hospitalisés. **Activités principales :** - Orientation et accompagnement des patients - Élaboration et mise en œuvre de projets individuels - Médiation avec les familles et partenaires extérieurs - Préparation des sorties et admissions en structures - Information sur les droits et accompagnement pour les faire valoir - Coordination avec les institutions et rédaction de rapports.
