Assistant(e) social(e)

Nous recrutons pour un établissement partenaire un(e) Assistant(e) de Service Social en psychiatrie. Votre mission principale sera d'évaluer et d'accompagner les patients dans leurs démarches médico-sociales, leur projet individuel et leur réinsertion. Vous travaillerez en collaboration avec l'équipe médicale et soignante pour maintenir ou favoriser l'autonomie des patients. Vos activités incluront la reconnaissance des problématiques médico-sociales, le conseil et l'orientation des patients, la gestion des dispositifs administratifs, l'accompagnement dans les formalités sociales et la visite à domicile. Vous devrez aussi former et évaluer les étudiants CESF et/ou assistant de service social. Des capacités d’adaptation, d’organisation, et de travail en équipe sont indispensables, ainsi qu’une bonne connaissance de la législation sociale.
